Job Duties
Monitors and provides support to patients and facilitate various recovery-oriented activities.
- Facilitates or assists with adjunct groups/meetings and performs all associated duties, including documentation.
- Provides support to patients during evenings and weekends, and when needed, help to de-escalate any emotional reactions patients might have.
- Collects appropriate data on bio-psycho-socials.
- Completes initial family contacts and provide information on family services and document appropriately and facilitates sign-up for Family Day Program and monitor family visitations.
- Provides new patients with general orientation to the facility, including but not limited to, introducing new patient, initial assignments, buddy system and introduction to the facility rules and regulations.
- Monitors patients during free time and their preparation for bedtimes and conduct bed and room checks.
- Communicates significant events via communication log, patient charts and verbally to key staff.

Education
High School Diploma or Equivalent (GED)- (Required), Bachelor's Degree-Social Sciences (Preferred)Experience
