Emergency Department Certified Peer Specialist - Weekend Days
Job Summary
Join our new and innovative program to assist patients in behavioral health crisis and reduce or avoid an inpatient psychiatric admission. Schedule: Weekend day shift; typically 2 shifts per month REQUIRED: Certified Peer Specialist (CPS) and 1-year related experience
Job Duties
The Certified Peer Support Specialist uses their own unique, life-altering experience in order to guide and support others who are in some form of mental health recovery. Peer Support Specialist works in conjunction with highly trained and educated professionals. They fill a gap by providing support from the perspective of someone who has first-hand experience, something that professionals cannot learn from training or education.
- Assists in the outreach and engagement of members identified as needing behavioral health support and treatment.
- Accompanies members to a variety of services or community support events in a variety of settings.
- Engages members by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
- Assists members with identifying recovery goals and strategies to achieve their goals.
- Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
- Participates in multidisciplinary treatment team conference to maximize the treatment plan.
- Provides feedback to the treatment team regarding members and offering a unique perspective.
- Assist members with identifying natural supports and resources in their communities and teach members how to utilize these supports.
- Performs appointment scheduling and clerical duties as needed.
- Builds relationships within the community to identify resources that are available to the patients i.e.: Targeted Case Management providers, outpatient psychiatric providers, community resources, etc.
- Promotes community integration through the connection of resources, linking to supports, mutual-help groups, social clubs, volunteer and job opportunities.

Education
High School Diploma or Equivalent (GED)- (Required), Bachelor's Degree- (Preferred)Experience
