LPN - Licensed Practical Nurse - Pediatrics Case Manager - Special Needs Unit
Job Summary
A Special Needs Unit (SNU) LPN Case Manager is a healthcare professional who is responsible for providing support and patient-centered care management services to members with Special Needs. The goal of this role is to ensure that all members with special needs are able to receive necessary services and supports in a timely manner. Assists members/families to navigate all aspects of care with a specialized focus on complex pediatric and pregnant members.
Job Duties
- Performs onsite and telephonic review to evaluate needs and determine appropriate management.
- Meets regulatory requirements related to management of Shift Care members including home support, Durable Medical Equipment, access to primary and specialty care, medication review, childcare access, transportation, financial strain, and housing.
- Serves as single point of contact for navigating health system and health plan needs.
- Provides culturally competent, evidence-based management to pregnant members.
- Partners with Maternal Home Visiting programs to facilitate individualized, strength-based, and family-focused care.
- Fosters a team approach and serves as a liaison between the member, family, treatment team, and community to ensure that all needs are being met and Social Determinants of Health are addressed.
- Collaborates with school personnel to ensure that the child’s educational needs are being met.
- Escalates and communicates worsening condition or urgent needs to appropriate care team members.
- Keeps detailed records of all contacts made with the child and family, as well as any other pertinent information.
- Works directly with Care Coordination team to implement processes to maintain/improve quality.

Position Details
This position is embedded in the clinic.
- Hours: 8 AM to 4:30 PM
- Travel: Less than 25%. May need to travel within Montour, Northumberland, Columbia, and surrounding counties for patient home visits. Travel may be required for meetings as needed.
- Required Experience: 5 years of LPN experience with a minimum of 2 years of Pediatric experience required.
