RN - Registered Nurse Coordinator - Transplant

Job Duties
The RN Nurse Coordinator serves in an expanded role to assess adult and pediatric pre- and post-transplant patient needs. Primarily responsible for coordinating clinical aspects of patient care. Works with patients and their families beginning with the evaluation for transplantation and continuing through and after transplantation. Serves as a resource person in establishing nursing guidelines and implementing nursing care.
This role is full-time, 40 hours weekly; hours are typically 8:00 a.m.- 4:30 p.m. during the week. This role does include on-call, which will be discussed during the interview.
A minimum of 5 years RN work experience is required. Previous experience in the inpatient transplant, nephrology or Dialysis is preferred, but not required.

Job Duties:
- During the pre-transplant process, explains to patients approaching end stage renal disease about the treatment modality of transplantation.
- Schedules and presents family conferences for potential transplant patients.
- Schedules tissue typing.
- Assesses health status of patient through history and referring physician and unit evaluation.
- Assists in screening per protocol and coordinates evaluation of the pre-transplant patient.
- Obtains, collates and reviews data necessary for presentation to Transplant Committee.
- Prepares patient for transplantation per recommendations of Transplant Committee.
- Coordinates annual evaluation of patients awaiting transplantation.
- Initiates and completes living related donor evaluation.
- Arranges for admission of cadaveric transplant recipient.
- Coordinates, collates and reviews preoperative evaluation.
- Reviews preop teaching with patient and family.
- Assembles lab data and records on transplant flow sheet.
- Collaborates with nursing personnel regarding plan of care, follow-up and evaluation.
- Coordinates requests for consultation with services including dietary and social services.
- Reviews and coordinates the need for laboratoryand othertesting.
- Reports pertinent occurrences to chief of staff, house staff and nursing staff.
- During the post-transplant process, obtains and reviews lab information, vital signs and nursing assessments prior to physical examination.
- Reviews current therapy with patient.
- Refers patient to ancillary staff as appropriate.
- Acts as point of contact for transplant patient and family members, as required.
- Provides patient teaching as needed in post transplant course Maintains call availability for patients and physicians.
- Assists in maintaining records for required transplant registries.
- Participates in research projects.
- Assists with maintaining and following transplant data.
- Participates in educating referring dialysis staff and patients by annual visits to these sites.
