RN - Registered Nurse Community Outreach - Per Diem
Job Summary
The Community Outreach RN serves in an expanded nursing role as a member of a county Behavioral Health and Intellectual Disability Program. Assesses patient needs and provides or proposes appropriate care.
Job Duties
This role is per diem, an as needed schedule to support Geisinger Community Outreach. At least (2) years of RN work experience is required. Experience, interest and comfort level working with pediatric and adult patients with behavioral health conditions is beneficial. This role supports Northumberland County Community Outreach
- Under the clinical direction of the psychiatrist, renders patient assessment and care.
- Completes documentation in timely and thorough manner consistent with policy to include record review, mental status, assessment, patient education, plan, etc.
- Performs triage including determination of appropriate level of care, scheduling of patient appointments, referral, etc.
- Provides concise and thorough patient evaluations.
- Participates in decision making process regarding diagnosis, treatment modalities, disposition planning and level of care.
- Assists in development of clinical services, operations, policies and procedures.
- Assists in the coordination of clinical services with other similar programs and services, external agencies or other providers.
- Manages flow of patient phone calls concerning patient condition and care, medication needs, etc.
- under the direction of psychiatric physician staff.
- Assists with telephone consults to staff from internal and external physicians and providers.
- Manages medication supplies including pharmaceutical company indigent programs.
- Conducts self in professional matter, upholding program policy and standards, as well as ethical standards of appropriate discipline.
- Renders patient and family education as appropriate.
- Assists in educational instruction of students, staff and other professionals.
