Job Duties
The Regional Director of Care Management provides direction and leadership to the continuum of care management program. Responsible for the daily operations of all functional components of staff competency, software systems support, performance-reporting, interface with other clinical enterprise departments, policies and procedures. Responsible for compliance with all relevant regulations for interaction with all associated regulatory agencies. Provides administrative and managerial direction for department. Establishes an integrated approach and use of clinical and financial data in program design, implementation and evaluation.
This role is full-time, Exempt (salaried), Monday through Friday with some weekends as needed. A Bachelor's Degree is required. An RN in the State of Pennsylvania is required. Five (5) years related work experience is required; previous managerial/supervisory experience is strongly preferred
- Contributes to the design and development of Care Management programs in coordination with division leadership.
- Oversees recruitment and retention of all professional and paraprofessional staff.
- Maintains operational responsibility for staffing plans to support the department including position descriptions, performance evaluations, action plans, employee goal setting, hiring, promotion and disciplinary actions.
- Assists with annual budget preparation for capital, manpower and operations funding.
- Ensures that the department operates in a cost effective manner Contributes to the establishment and delivery of initial and ongoing staff education programs.
- Maintains compliance with all relevant regulations as related to the department; specifically, in the areas of consistent standards of care, documentation, and consistency in decision making.
- Establishes collaborative working relationships within and outside the organization.
- Ensures appropriate and accurate documentation of all serviced ordered and rendered on patient medical record.
- Discerns content, manner and timing of information sharing with sensitivity to individual and organizational concerns.
- Responsible for hospital utilization functions-determination of appropriate clinical utilization.
- Responsible for monitoring and reporting trends in utilization.
- Responsible for improvement related to opportunities illuminated by trended data.
- Integrates this function and data with all other pertinent hospital processes and functions.
- Responsible for all related regulatory compliance.
- Supervises department staff.
