Nurse Practitioner or Physician Assistant Skilled Nursing Facilty Per Diem
Job Summary
Geisinger is seeking an experienced Nurse Practitioner or Physician Assistant for a per diem position in various skilled nursing facilities as needed.
Job Duties
This position requires an experienced advanced practitioner, prior experience working in a skilled nursing facility (SNF) is helpful but not required. The SNF advanced practitioner (AP) provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.
Overview:
- Hours: Monday – Friday 8 a – 4:30p
Responsibilities include:
- Communicate with SNF staff and review patient reports
- Treat acute needs, admissions, discharges, etc.
- Manage complex and non-complex patients
- Average 12 patients a day
- Length of time and number of patients as per need

- Oak Ridge Rehabilitation and Healthcare Center, Scranton
Position Details
Family Nurse Practitioner, Adult – Geriatric Nurse Practitioner or Physician Assistant with advanced practice experience in a skilled nursing facility or long-term care facility is preferred.
Education
Master's Degree-Nurse Practitioner (Required)Certification(s) and License(s)
