Full Time Monday - Friday 8am - 4:30pm; REQUIRES: High School Diploma or GED and active Certified Recovery Specialist (CRS) credentials; previous related experience strongly preferred
Job Duties
The Certified Recovery Specialist (CRS) is a member of a multidisciplinary team working to address the disease of addiction within treatment programs. Works within the community, providing outreach to patients with complex addiction, behavioral and psychosocial needs. Acts as a resource to assist patients in navigating the health care system, and motivating them to engage and sustain engagement with the behavioral and addiction treatment system.
- Assists in the outreach and engagement of patients identified as needing substance abuse support and treatment.
- Conducts regularly scheduled meetings with patients and appropriately engages them to identify interests, strengths, goals, dreams and aspirations while offering encouragement and empowerment through shared experience to enhance strengths and capabilities for members.
- Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
- Facilitates adherence to treatment plan by early identification and resolution of gaps in care.
- Supports patients in planning for and attending Step Meetings, finding a Sponsor and doing Step Work when appropriate.
- Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
- Participates in multidisciplinary treatment team conference to maximize the treatment plan.
- Provides reinforcement of the treatment plan under the direction of the Addiction Coordinators.
- Appointment scheduling and clerical duties as needed.
- Promotes community integration through the connection of resources, linking to supports, mutual-help groups, social clubs, volunteer and job opportunities.

Education
