Job Summary

Facilitates change and takes shared accountability with operational and clinical staff to achieve transformational results at the project level. **This is a hybrid position - candidates will have the opportunity to work remotely but will be required to perform clinic site visits every 8 weeks**

Job Duties

  • Manages the work to meet project milestones through task management, consistent follow through, relationship management and creative organizational problem solving.
  • Consistently achieves project outcomes at target goals and reaches stretch goals frequently through consistent follow-through, creative problem solving, removing barriers and tenacious focus on results.
  • Hardwires process improvements and embeds process and outcome metrics in improvement initiatives that draw data electronically from clinical and administrative records.
  • Formulates improvement strategies and approaches based on understanding of IT infrastructure and business needs, with particular focus on optimizing use of the electronic medical record and clinical system applications related to the delivery of patient care.
  • Identifies issues related to complex requirements including data storage, retrieval and interface needs.
  • Assesses the local stakeholder environment and builds the support and will to address improvement opportunities among line staff, department managers, vice presidents, physicians, physician program leaders and department managers.
  • Establishes and manages relationships to achieve transformational change with line staff, physicians, physician program leaders and physician chairs.
  • Facilitates and trains interdisciplinary groups and is able to support manager and physician in leading interdisciplinary committees.
  • Uses analytic and subjective assessment skills to identify substantial performance improvement opportunities at the project level.
