Population Health Nurse Practitioner or Physician Assistant
Job Summary
Geisinger Population Health team is seeking an experienced Nurse Practitioner or Physician Assistant to join our team in this fully remote position.
Job Duties
What You'll Do:
Focus on several key areas in the management of complex patients who are in the Accountable Care Organization (ACO)
Combination of virtual Annual Wellness Visits, virtual Transitions of Care on complex patients, and virtual acute visits
Work alongside a team of case managers, social workers, and community health workers as a partner with primary care providers
Requirements:
Certified Nurse Practitioner or Physician Assistant who resides in Pennsylvania with an active Pennsylvania license
Experience with telemedicine/virtual care is required
Experience managing patients who are both medically and socially complex is required
Exposure to and familiarity with an Accountable Care Organization and value-based care is preferred
Ability to work both independently and as part of a collaborative team
Schedule:
This is a fully remote position

Bachelor's Degree-Physician Assistant (Required), Master's Degree-Physician Assistant (Preferred)Certification(s) and License(s)
