RN - Registered Nurse Infusion - Community Based - Per Diem

Job Duties
This RN position is per diem, as needed. Typical hours are 8am to 4:30p or 8:30am to 5pm. No weekends or holidays.
The ideal candidate must be able to travel within a designated service area, which will be determined during the interview process based on their location.
At least three (3) years of RN work experience is required.
IV, Infusion, and Home Health experience is helpful.
The RN in Home Infusion provides community-based infusion nursing services to patients as prescribed by their physician. Assess, plan and teach patients from a broad age range and diverse backgrounds about the infusion, the therapy and the equipment required to provide infusion services.
Position Details
Home Infusion Nurse is responsible for coordinating and delivering comprehensive infusion therapy services in both home, suite, and other clinical settings. This role involves managing referrals, verifying clinical and administrative requirements, and providing direct patient care, education, and case management. The nurse collaborates with physicians, pharmacists, and interdisciplinary teams to ensure safe, effective, and patient-centered care throughout the continuum.
- Coordinate intake and referral processes, verifying demographics, clinical history, authorizations, and consents.
- Develop and implement individualized care plans based on thorough patient assessments, including venous system evaluation.
- Prepare and administer infusion medications using aseptic technique; perform vascular access procedures and lab draws.
- Educate patients and caregivers on therapy protocols, equipment use, and vascular access care.
- Monitor therapy progress, adjust care plans, and communicate with providers to ensure optimal outcomes.
- Coordinate care and communicate effectively with providers, pharmacists, internal departments, and external healthcare teams to ensure continuity and quality care.
- Maintain accurate documentation in the EMR and ensure compliance with clinical protocols and accrediting standards.
- Respond to patient and caregiver concerns, escalating issues appropriately to ensure resolution.
- Participate in the development of infusion protocols, patient education materials, and quality improvement initiatives.
- Supports 24/7 care coverage through participation in on-call rotations, including weekends and holidays.
- Collect and analyze clinical data to inform care decisions and contribute to the evaluation of advanced care models.
- Travel to patient homes and clinical sites to deliver infusion therapy, provide infusion related patient care, and perform patient teaching/assessments.
- Provide clinical oversight and guidance to Licensed Practical Nurses, ensuring adherence to care standards and supporting professional development.
- Other duties as assigned.
