System Associate Vice President- Inpatient Care Management- Care Without Delay Program
Job Summary
Provides leadership and oversight, guiding strategic development, planning, implementation, evaluation, and continuous improvement within the inpatient care management space. Ensures seamless transitions between inpatient care management, outpatient case management, post-acute care networks, and affiliate hospitals. Collaborates with various stakeholders, including hospital and health plan leaders, administrative leaders, inpatient and ambulatory care teams, pharmacy, post-acute care providers, revenue cycle leaders, and legal and regulatory departments. Maintains high standards of care and monitoring data metrics to achieve organizational goals and objectives.
Job Duties
- Develops system, regional and hospital-based strategies to implement the components of Care Without Delay.
- Observes the components of Care Without Delay and works collaboratively with leadership to assure compliance.
- Assures alignment of goals and sharing best practices across the enterprise.
- Maintains system and processes for effective quality and clinical outcomes management programs.
- Monitors the metrics and compliance for the various Care Without Delay components.
- Works with various healthcare providers, community organizations, and support services to ensure patients receive the necessary follow-up care.
- Develops, coaches, and manages a staff within scope of accountability to ensure consistent provision of high-quality clinical care.
- Oversees the complex care management team across the organization.
- Develops strategies and strategic partnerships to assist the complex care management team with challenging patient placement.
- Partners with physicians and clinical teams to make recommendations for placement options based on the care needed.
- Serves as a subject matter expert in the care management discipline to assist with the continuum of care.
- Identifies key performance metrics for department of care management and monitors accordingly.
