Registered Nurse Healthy Beginnings
Job Summary
Healthy Beginnings Plus is an enhanced, comprehensive package of services developed by the Department of Human Services to assist low-income, pregnant women who are eligible for Medical Assistance to have a positive prenatal care experience. It includes, in addition to medical services, a variety of health promotion and counseling services, as well as home and community-based services.
Job Duties
The Healthy Beginnings RN performs client assessment and triage including coordinating referrals to in-house clinical and community services, conducting family education, providing all aspects of nursing care, assisting physicians and physician extenders. Ensures that the client receives the mix of services appropriate to her needs in a timely fashion. Provides primary clinical support to the provider, patient and their family to assist in the diagnosis and treatment of the patient's condition. Follows up on diagnostic testing to ensure completion, accuracy, and filing of same in medical record to ensure continuity and quality of care.
This role is full-time, 40 hours weekly; 8-hour shift, weekdays with hours between 7 am to 5:30 pm. A minimum of 1 year RN experience is required.

Job Duties:
- Enrolls client into Healthy Beginnings Program and oversees the overall care of each client.
- Serves as the client's advocate, enabling service and providing follow-up.
- Assures client prenatal record is available at each visit and is accessible to all health care staff members involved in client's care.
- Initiates prenatal history and assists provider with physical.
- Develops care coordination record, initiating appropriate referrals.
- Maintains comprehensive problem list and updates at each visit being cognizant of all aspects of the client’s care.
- Completes trimester education encouraging active participation of each client in the care process.
- Provides education to improve maternal and neonatal outcomes.
- Serves as a resource for the initiation of additional sites.
- Provides reporting to the regulatory agencies as required.
- Assists with coordinating external disease management and case management organizations.
- May provide addiction case management related support to patients.
