RN - Registered Nurse Navigator - Cardiology
Job Summary
Facilitates entrance of new patients into care programs. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
Job Duties
- Responsible for the coordination of pre and post hospital care.
- Serves as the liaison between the patient and caregivers.
- Coordinates appropriate multidisciplinary clinics.
- Performs duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the multi-disciplinary program.
- Facilitates entrance of new patients into care programs.
- Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.
- Provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.
- Collaborates with other healthcare providers in coordinating and implementing the patient's plan of care including evaluation, treatment, follow-up and surveillance.
- Communicates with patient via mail, telephone, and in person regarding information pertinent to their care Coordinates patient and interdisciplinary activities to meet the needs of a defined group of patients.
- Acts as an effective resource.
- Coordinat's patient activities through the interdisciplinary clinics as appropriate.
- Reinforces patient and family education based on individual needs either in person or by telephone.
- Coordinates patient activities through available support services, such as support groups and social services as appropriate.
- Maintains systems and processes to assure timely follow-up and surveillance of post-follow-up patients.
- Provides nursing care and assists physician and procedures.
- Educates other team members in area of expertise.

Position Details
This role is part of a highly specialized and cohesive clinical team, specifically aimed at treating patients with structural heart issues. The ideal candidate is someone who is dedicated to the training that will be involved with this role and who is willing to grow within the role!
Hours: 40 hours per week; Days
Experience: Minimum of 2 years RN work experience is required

Education
Graduate from Specialty Training Program- (Required), Bachelor's Degree-Nursing (Preferred)Experience
