Home Care Coordinator
Job Summary
Responsible for the development and implementation of home care services for participants, including the coordination of personal care services, durable medical equipment needs and nursing home care.
Job Duties
- Coordinates the implementation of all home care services, which include nursing, physical, occupational and speech therapy, personal care services, dietary needs and durable medical equipment, to ensure that quality service is provided to meet participant needs.
- Ensures appropriate authorization is obtained and entered into required software programs.
- Conducts initial nursing assessments and revaluations of participants, including home assessments in collaboration with social worker.
- Presents assessments to the interdisciplinary team at scheduled team meetings and collaborates on the creation and revision of care plan.
- Assesses home care services function, makes recommendations and offers solutions to revise or modify plans of care to better meet participant's needs.
- Manages staff roster and schedules for all home care on a daily, weekly, and monthly basis to create the most time efficient and cost effective schedule to meet the needs of participants.
- Prepares weekly seven-day home care schedule for coverage of personal care, companion and respite services.
- Supervises all personal care assistant staff including home and center based care.
- Coordinates scheduling for coverage of toileting, bathing, and assigning escorts to accompany participants for outings and appointments.
- Completes human resource management of assigned home care aide staff including hiring, annual performance appraisals, competency assessments, disciplinary actions, payroll issues, and educational monitoring.
- Provides routine nursing care for ongoing or episodic illnesses at home according to the plan of care.
- Provides counseling and education to participants and caregivers regarding physical, emotional and environmental health problems and chronic disease management.
- Records, maintains, monitors and verifies accurate home care records, including service documentation, attendance and payroll, in-service, medical records and billing according to program policy.
- Receives and follows up on concerns raised by customers related to home care services.
- Participates in on call coverage rotation with other members of the management team to troubleshoot, advise and coordinate participant care.
- Obtains and tracks equipment, supplies and services for home use by participants with the assistance of program clerical staff.
- Ensures routine maintenance of home equipment is completed as required.

Education
Graduate from Specialty Training Program-Nursing (Required)Experience
