Clinical Practice Transformation Coordinator
Job Summary
Leads efforts to align Value Based Healthcare and Best Practice processes through collaborative transformation efforts with clinical organizations and departments within Keystone ACO. Provides workflow analysis and collaboration to initiate implementation plans within the Keystone ACO clinical sites. Develop best practice knowledge and use superior communications skills to complete training of implementation plans and practices to office staff at all levels. Provides guidance to the health care with access to care coordination activities including protocol-driven disease management services and education for Keystone ACO patients. Point person for Keystone ACO Practice Sites concerning Clinical, Administrative and IT implementation and transformation processes, including trouble shooting, process workflow analysis and assistance with process documentation. Takes the lead in patient and provider satisfaction evaluations efforts, reengineering activities and quality improvement projects as required. Develops and sets standards for documentation and collaborates with the health care team to adjust interventions to maximize clinical, quality, and fiscal outcomes.
Job Duties
- Drives the design of interventions to support the implementation and transformation of Value Based Healthcare processes through evidence-based medicine.
- Coordinates with the provider, and other health care team members to implement patient specific strategies to enhance chronic care and preventive care management.
- Creates and utilizes established education and clinical pathways to support comprehensive and consistent disease management education to close patients care gaps.
- This includes the collaboration with primary care and specialty providers as well as the health care team to enhance clinical guideline adherence and promote best practice by initiating pre-visit planning and preventive and specialty care education and scheduling services when appropriate.
- Develops recommendations for implementation based on analysis of clinical practice site workflows, staffing levels and procedures throughout Keystone ACO Organizations.
- Utilizes Care Gap outcomes to promote benefits and availability of disease and case management services with providers and other health care providers.
- Identifies problems, develops process improvement strategies and assists the Care Gap team in Quality Improvement and reengineering activities.
- This includes identification and development of key clinical initiatives to improve quality and workflow to enhance clinic efficiency and patient engagement.
- Leads implementation of Clinical Protocol and care gap projects as directed.
- Leads product integration as assigned by Keystone ACO Leadership.
- Collaborates with various departments within Geisinger and Keystone ACO to obtain subject matter expertise and to garner resources to move ideas forward.
- Analyzes, develops and conducts training needs for users of assigned clinical service lines as related to workflow.
- Conducts workshops and provides individual support for clinical operations as required including shadowing when appropriate.
- This position serves as a liaison and translator between clinical operations and Information Technology to facilitate change.
- Provides linkage to the Medical Neighborhood including but not limited to Emergency Rooms, and Skilled Nursing Facilities.

Education
