Respiratory Therapist Navigator Registered
Job Summary
Utilizes clinical judgment and expertise to coordinate care management activities for adult pulmonary patients. Performs patient assessments, develops and implements treatment plans, provides education to both patients and their families, and completes discharge bundles prior to discharge and ensures all components of transitions of care are complete. Works closely with the pulmonary team through rounding and daily communication. Serves as a liaison between pulmonary medicine and the inpatient respiratory therapist. Facilitates seamless care for adult pulmonary patients, both outpatient and inpatient, and their families, and ensures effective and timely use of resources to address identified needs. Promotes quality care delivery across the continuum of care. $15,000 Sign-On Bonus for eligible applicants!!
Job Duties
- Assists physicians with evaluations, treatments and diagnostic procedures.
- Performs and documents thorough patient assessments and therapeutic modalities.
- Performs and documents routine therapeutic modalities including aerosolized pharmacological agents, secretion management techniques, pulmonary volume expansion, and oxygen therapy.
- Stays informed of evidence based practice models for patient care and treatment plans and integrated them into patient centric treatment plans.
- Reviews respiratory treatment plans with patients and families.
- Serves as a strong role model in professional practice as evidenced by strong interdisciplinary relationships, effective communications, appropriate resource utilization and sound planning skills.
- Suctions and maintains airways as needed.
- Rounds with pulmonary team for on the spot treatment plan modifications and timely communication to inpatient respiratory team.
- Provides age appropriate and specific patient and family education, respiratory therapies, medications and airway clearance techniques.
- Serves as liaison between inpatient and outpatient to standardize patient and family education and teaching methods.
- Assists with clinic to hospital transfer of admitted patients as necessary.
- Assists with discharge planning for patients needing respiratory related Durable Medical Equipment supplies.
